• Weekly Meal Stipend: $413 *Rates may vary based on stipend needs* Position Description:Number of Beds : 400 Shift: Mixed (day 8 am-430 pm, 3-1130 pm, 630-1900, 10-2230, 1830-0700) Patient Ratios : 1-25 Scheduling requirements: 6 weeks in advance Weekend Requirement: as needed Hospital Specifics-Certifications: Current RN License Must have: Current RN License CCM preferred Bachelor s Degree preferred minimum of one year acute care or payer case management, utilization management, discharge planning, resource management experience required. Minimum of three years of acute care nursing required. Proficiency with information systems and computer systems. MIDAS preferred. Advanced skills needed: understanding of utilization management, case management, health care financing, alternative care options, goals, outcomes, regulatory and professional implications of practice. Procedures performed on unit: assessing, planning, facilitating, and advocating for options and services through the continuum of care from point of contact through discharge on assigned patients. Case Manager promotes quality and cost effective outcomes. Emphasis is on care coordination, communication, and collaboration with nursing, physician, departments within the facility, insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care. The Case Manager consistently conducts the utilization review process on those patients who require it, and the discharge planning process as required by hospital policies, standards of practice, and Federal and State regulations. Responsibilities for this Case Manager opportunity include, but are not limited to:
• Responsible for the coordination of health care decisions by using a systematic approach to ensure treatment plans that improve quality and outcomes
• Responsible for the promotion of cost-effective care within allotted time frame
• Assure payments of hospital-based services meet patient-related utilization of management criteria
• Implementation of safe and appropriate discharge plans
• Asses the Case Management needs of the patient and provide intervention as part of the discharge planning process
• Primary functions include: Clinical Interventions/Discharge, Care Planning Management, Fiscal Manage and Payer/Referral Management JOB REQUIREMENTS FOR THE RN CM REGISTERED NURSE CASE MANAGER:
• Current licensure as Registered Nurse in state of practice
• Previous experience in case management or related area
