Job Description:
HealthTrust Workforce Solutions HCA is seeking a local contract
nurse RN Case Management for a local contract nursing job in
Riverside, California.Job Description & Requirements
- A recent 3 years of acute nursing experience and a recent 12
months as an RN Case Manager or payer case management, utilization
management, discharge planning, or resource management
experience.
- A CA RN license (CCM and Bachelor's Degree preferred)
- BLS certification
- Advanced Skills Needed: Advanced skills needed: understanding of
utilization management, case management, health care financing,
alternative care options, goals, outcomes, regulatory and
professional implications of practice.
- Procedures Performed on Unit: assessing, planning, facilitating,
and advocating for options and services through the continuum of
care from point of contact through discharge on assigned patients.
Case Manager promotes quality and cost-effective outcomes. Emphasis
is on care coordination, communication, and collaboration with
nursing, physicians, departments within the facility, insurers, and
post-acute service providers to pace the care toward optimal
outcomes within the appropriate level of care. The Case Manager
consistently conducts the utilization review process on those
patients who require it, and the discharge planning process as
required by hospital policies, standards of practice, and Federal
