Job Description CHHA (Certified Hospice Aide): Performs various services for patient as necessary to meet patient's personal needs and to promote comfort. The hospice aide is responsible for patient care and report any observation to supervisor. LVN (License Vocational Nurse): Responsible for providing direct patient care. LVN responsivity include follow care plan, provide treatments as well as work together with other team members to meet patients care needs. RN (Register Nurse): Plans, organizes and directs hospice care, RN must have the experience and education to direct hospice care. The professional nurse builds from the resources of community to plan and direct services to meet the needs of individual and families with in their home communities.
