Peds case in Riverside Monday-Friday 9am-5pm Position Description:The Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) assumes responsibility andaccountability for the application of the nursing process and the delivery of patient/client care and/orcontracted services under the supervision of a registered nurse or licensed designee. The LVN/LPNprovides service in accordance with Maxim philosophy, standards and policies.1. Utilizes the nursing process to plan, implement, and evaluate patient/client care and/orcontracted services2. Demonstrates on each assignment the provision of skilled nursing care rendered in accordancewith prescribed orders3. Identifies and documents changes in patient's both physiologic and psychosocial condition4. Notifies Director/Supervisor if applicable on the following;a. Physician notification to obtain necessary orders for intervention(s) per stateregulationsb. Changes in patient/client condition5. Adheres to Maxim's policies and procedures and applicable protocols6. Identifies and prioritizes actions based upon patient/client care requirements7. Assures patient safety at all times and recipient of services provided8. Verifies that documentation reflects patient/client collaboration, participation in plan of care,expected patient/client outcomes, and selected nursing interventions 9. Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies and patient/client care outcome standards as applicable. Monitors and documents effectiveness of interventions, treatments, and medications 10. Evaluates patient/client and provides a safe, comfortable and therapeutic patient/client environment 11. Monitors the safety and effectiveness of the environment and equipment including universal precautions 12. Participates in activities to facilitate good interpersonal communications 13. Communicates observations and significant changes in patient/client status to the Director of Clinical Services and/or Clinical Supervisor /Case Manager or licensed designee 14. Documents all physician collaboration/coordination of care in the medical record andcommunicates this information as needed15. Attends case conferences, team meetings, education in-services as required16. Supports professional practice by observing the following;a. Nurse Practice Actb. Standards of Nursing Practicec. Standards of Patient Cared. Standards of Accrediting Body (ACHC, Joint Commission, etc.)e. State and Local Lawsf. Patient Bill of Rightsg. Maxim Healthcare, Nursing, and Client's Hospital Policies17. Performs other duties as assigned Minimum Requirements: 1. Graduate of an accredited practical nurse or vocational nursing program, preferred 2. Licensed Practical Nurse or Licensed Vocational Nurse in specific state 3. Complies with accepted professional standards and practice 4. At least one year prior professional nursing experience, preferred 5. Willingness to participate in skills advancement training or novice nurse program if applicable 6. Current CPR as applicable, for direct patient care 7. PPD or chest x-ray if applicable 8. Successful completion of new hire training as applicable to job site 9. Participation and completion of Maxim's Competency Program 10. Strong verbal and written communication skills 11. Current Health Certificate (per facility contract or state regulation) 12.
