Job Description:
American Veterans Hospice is locally operated and owned by a
Veteran. Our Agency is dedicated to providing the highest quality
care to all patients and the most rewarding experience to all team
members. We serve those who are in the final stages of their
journey while honoring their life. We are searching for in home
hospice experienced Admissions nurses who have a deep understanding
of LCDs and Hospice Eligibility Criteria (PPS, FAST, NYHA). Our
search is for nurses who are open to any of these areas: Riverside
County, San Bernardino County.Duties and Responsibilities:
The hospice admission nurse is an
experienced registered professional nurse who initiates and
coordinates the hospice care plan, provides skilled and palliative
care and coordinates the services provided to the newly admitted
patients.Provide comprehensive nursing evaluations and assessments
to develop a detailed Plan of Care for assigned patients ensuring
all services needed are identified and met while working closely
with the Interdisciplinary Group, patient, family and attending
physician, including working with CTIs.Documents assessment,
current medications and initiates appropriate care plan in Agency
EMR (Hospice MD).Responsible for assessing patients clinical
status, planning, teaching and implementing immediate interventions
on the day of admission.Communicates admissions information,
issues, and concerns in patient care meetings to assure smooth
transition and continuity of care with other assigned staff,
including a detailed report to the assigned RN/Case Manager, Social
Worker, and AdministratorUtilize interpersonal skills to help
appropriately educate and address any questions the patient and/or
patients family has concerning hospice care.Obtains informed
consents, provides nursing assessment, initial plan of care and
intervention(s) that optimize patient and family directed comfort
and quality of life. -Qualifications:
RN - (Registered Nurse) licensed in
good standing and actively practicing in the state of
CaliforniaPrevious experience min 1 year+ as Admissions Nurse in
hospice. Expertise with LCDs and Hospice Eligibility
Criteria.Detailed oriented, skilled written and oral communicator,
organized and excellent coordination skills Electronic Medical
Record experience (Hospice MD, etc)Demonstrated commitment to
Agencys philosophy of care, values, mission statement and
organizational culture.Committed to improvement, clinical
excellence, high-degree of patient and staff satisfaction.Prolonged
or considerable walking or standing. Able to lift, position, and/or
transfer patients. Able to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling, and/or
crouching. Visual acuity and hearing to perform required nursing
skills.Must have reliable automobile, valid drivers license, and
the minimum state required liability auto insurance.Able to travel
to multiple locations in the course of the working
day/week.Demonstrates excellence in customer service Updated
