From heartfelt conversations to creating personalized care plans, you'll lead the way in making sure every patient and family feels supported, informed, and cared for. What You'll Do: Compassion Meets Action : Perform comprehensive assessments, guide care plans, and work directly with physicians to ensure each patient's needs are met. Documentation Extraordinaire : Keep your notes sharp—EMR documentation within 24 hours! On the Move : Travel to patient homes (yes, this includes navigating stairs and weather surprises) to deliver exceptional care. Must be willing to travel to Orange County and Inland Empire. Family Whisperer : Educate, comfort, and empower families during their hospice journey. Teamwork Makes the Dream Work : Collaborate with our interdisciplinary team to achieve positive outcomes. What You Bring to the Table: ?? Graduate of an accredited nursing program. ?? RN License (active and in good standing). ?? 1 years of nursing experience (hospice experience required). ?? A valid driver's license, insured vehicle, and a sense of adventure for the road ahead. ?? Bilingual (Spanish/English)? Amazing!
