VISTA is assisting a health care organization that is fast growing and highly dynamic in the recruitment of an Extensivist. You will be joining a platform that enables health care systems and providers to become population health leaders. A few details of the employed, full-time opportunity: "Seeking a BE/BC Extensivist to provide services to the senior patient population across multiple settings "A partner for building a smarter and better healthcare systems for communities by integrating with hospitals, physician groups, health plans and employers "Advanced clinical model is designed to lower cost and increase care quality to create value for the patient with a focus on inpatient prevention "Travel locally between care centers daily to treat and manage patients (high risk, frail) in hospitals, home visits, and care centers "Communicate daily and coordinate care plans with covering hospitalist, specialty teams, managers and primary care providers. "See, treat and manage 'skilled' patients in the SNFs according to established protocols. These patients are seen at least twice a week until they remain skilled, which is normally from 1 to 2 weeks. "Supervise, mentor and assist Nurse Practitioners as needed. "Participate in a weekly complex case management multidisciplinary meeting "Schedule: M-F 8am - 5pm; 8am - noon rounding in hospitals; on-call schedule as needed "Low patient census of up to 10 patients per day; 20-40 minute consults per patient "IM or FM Residency trained required and 2-3 years of Hospitalist, Family Practice, or Internist experience preferred, but will also consider graduating residents and fellows "Salary: starting $200-250K base, plus bonus potential: bed days, clinical performance, low admission rate "Full benefits, CME reimbursement up to $1500 for courses and conferences, relocation, interview travel fees covered, licensing fees "San Jose, Riverside, ...
