receive care. SUMMARY: Manager with a health care system
development and integration for the organization will be
responsible for improving membership retention, quality of care,
health outcomes, and regulatory compliance as it relates to Quality
Management and Case Management programs. Oversight and involvement
of all Case Management components, including Inpatient Case
Management, Chronic Care Case Management, and Transitional Case
Management; as well as ACO, Population Health, and Skilled Nursing.
Ensures staff participation in MDT, oversight of patient
utilization, including bed days (acutely and post-acutely),
aberrant days, transition to post-acute settings, rounding at acute
and post-acute facilities where RMC patients are admitted.
Oversight with Ambulatory services, as mentioned above, to assure
continuity of care. Assists with implementing strategies for best
practices. Preparation and participation in health plan audits
entailing Case Management components. QUALIFICATIONS: To perform
this job successfully, an individual must be able to perform each

HOURS: Monday - Friday; 8:30am - 5:30pm
Qualifications
EDUCATION and/or EXPERIENCE: Associate's Degree from an accredited
college or two years college preferred. Four years related
experience required with three to five years of program
development, and Quality required. Must possess knowledge of
Grievance Appeals functions, Accreditation, and knowledge of
regulatory standards issued by CMS and DMHC required. CERTIFICATES,
LICENSES, AND REGISTRATIONS: Current, unrestricted California LVN
