Job Description:
A company is looking for a Care Coach to deliver holistic care
to patients with chronic neurological conditions.
Key Responsibilities:
Meet regularly via telehealth with patients to guide them towards
health goals across various aspects of wellness
Utilize behavioral science techniques to support patients in
overcoming barriers to their health goals
Document coaching visits accurately and collaborate with the
broader care team
Required Qualifications:
2+ years of health and wellness coaching experience
NBC-HWC Certification
Proven track record of helping clients achieve health goals
Experience with motivational interviewing and behavioral coaching
techniques
Tech-savvy and comfortable working in a digital environment
